For Our Patients

Welcome to the Neurosurgical Group of Chattanooga. We are pleased to assist your primary medical doctor with treatment of your neurosurgical condition. We hope to provide your medical care in a safe, confidential environment, and strive for a positive outcome. To that end, we ask for your help and cooperation in your diagnosis and treatment.

Appointments:

We will make every effort to schedule and see you as soon as possible. On the day of your appointment, we ask that you arrive 15 minutes early to fill out any additional required paperwork and in case there are any problems with certification of your health insurance. If you are unable to keep your appointment, we ask that you call and notifiy us as soon as possible so that we may assign that time slot to another patient who needs to be seen in the clinic.

First Visit:

For your first visit, please do the following:

Arrive 15 minutes early for your appointment
Bring your current insurance card with you
Your co-payment will be due at this visit, so please be aware of the copayment
Bring all MRI scans, CT scans, and other medical studies. The typewritten report of these studies WILL NOT be sufficient to accurately diagnose and treat you. In all cases, we must be able to view the actual medical images.
If you do not have insurance, please be prepared to pay for your visit at the time of services.
For your convenience, you may download and print our Patient Health History Form prior to your arrival. You will need to fill out this form prior to being seen by the doctor. If you do not have access to a computer or printer, our office can mail you an intake form. Otherwise, one will be provided for you upon arrival to the office.
